Registration
All participants must be registered and are urged to register in advance. You can register either:
- Download the registration form on PDF format, fill it and send it along with your payment to the congress office.
- on line on this secure server
Registration per phone will not be processed. Registrants will receive confirmation by way of an invoice and acknowledgement letter which will be mailed and which will recall all the information given on the registration form, including the amount of Value Added Tax (VAT) paid and all other requests. This confirmation letter should be presented upon arrival at the Preregistration Counter if the account is settled or at the on-site registration if a balance is still due.

Delegates’ registration fees include:
- Admission to all congress sessions,

Terms of payment:
The Associations ADECRO (Hôpital Cochin, Pavillon Ollier, 27 rue du Fbg St Jacques, 75679 Paris Cedex 14) et ACORA (116 rue Antoine Charial, 69003 Lyon) have given mandate to MCI for the organization of the 5th International Symposium “Charnley Evolution 2009” and therefore to act and receive the fees on their behalf.
Payment must be sent to the Congress Office in EUROS (€)
- by cheque payable in France to the order of Charnley 2009 – MCI France
- or by Credit card : VISA, EURO/MASTER CARD or AMEX only (NO other cards accepted)

Copy of the bank transfer must be attached to the registration form. Do NOT forget to give the name of the person you are paying for, on the bank transfer order.
Name Changes
A processing fee of 30€ will be charged for changes in registration except for name substitutions which will be treated as new registrations.
Cancellation policy for individual registration & reservations
All cancellations must be notified in writing (by mail, email or fax) to the Congress Office according to the following conditions:
- Up to September 3, 2009: 100€ will be withheld for administrative fee
- Between September 4 and October 15, 2009: reimbursement of 50% of the fees paid
- From October 16, 2009: no refund.
Reimbursement will be processed AFTER the Congress.
Requests for refunds made after March 3, 2010 will not be considered.
